* A list of all your children and your beneficiaries, including their address, their birthday, and their relationship.
* Your driver’s license or state ID.
* All deeds to any property that you may own.
* A current tax statement with an address and parcel number.
You can also bring copies of your recent bank statements with their full account numbers and full address, all non-qualified and non-tax deferred account statements, stock and bond certificates, and life insurance policies should you decide to put those in. Thank you and have a great day!
